New York NCLEX Application – Nursing Step 1: Applying with the New York State Board of Nursing New York NCLEX Application: You will need to turn in a paper application. This is the New York State Board’s Homepage. All information here is taken from that source. Start Here: Go to the Licensure Forms/Application page and open ‘Form 1 – Application for Licensure.’ Follow its instructions closely. They will charge you $143 with an option of $35 for the temporary permit. This is the State Board Fee. New York Board of Nursing Contact:
